Research should specifically:<\/p>\n<ul>\n<li>Develop and test metrics for evaluating the impact of survivorship care planning<\/li>\n<li>Identify the impact of survivorship care planning on cancer survivors&#8217; morbidity, self-management, adherence to care recommendations, utilization of follow-up care and on systems outcomes<\/li>\n<li>Identify models and processes of care that promote effective survivorship care planning<\/li>\n<\/ul>\n<p>Awards last for up to five years and should reflect the needs of the project. Applications are due February 5, 2013.
